Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

American Institute of Mental Health Inc. ("we", "us") is required by law to maintain the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

For treatment. To provide, coordinate and manage your child's ABA services — for example, sharing the treatment plan with the BCBA and the RBT working with your child, or coordinating with your child's pediatrician or school with your authorization.

For payment. To verify coverage, obtain prior authorizations, submit claims and collect payment from your health plan.

For health care operations. Quality review, supervision of clinical staff, training, credentialing, audits and business management.

Appointment reminders and communications. To contact you about appointments, services and administrative matters, including by phone, email and, if you have opted in, text message.

Business associates. To vendors who perform services for us (billing, electronic records, secure hosting, telecommunications). Each signs a Business Associate Agreement requiring them to protect your information.

As required by law. Including mandatory reporting of suspected abuse or neglect, public health activities, health oversight, judicial proceedings, law enforcement in limited circumstances, and to avert a serious threat to health or safety.

Uses that always require your written authorization

Most uses and disclosures of psychotherapy notes
Use of your information for marketing purposes
Any sale of your protected health information
Publication of testimonials, photographs or case information

You may revoke an authorization in writing at any time. Revocation does not affect disclosures already made while the authorization was in effect.

Your rights

Get a copy of your records. You may inspect and request a copy of your child's records, electronically or on paper. We will respond within 30 days and may charge a reasonable, cost-based fee.

Ask us to correct your records. If you believe information is incorrect or incomplete, you may request an amendment. We may deny the request, and we will explain why in writing.

Request confidential communications. You may ask us to contact you in a specific way or at a specific address. We will accommodate reasonable requests.

Ask us to limit what we use or share. We are not required to agree, except that we must agree if you pay in full out of pocket for a service and ask us not to share that information with your health plan.

Get a list of disclosures. You may request an accounting of certain disclosures made in the six years before your request.

Get a paper copy of this notice. At any time, even if you agreed to receive it electronically.

Choose someone to act for you. A parent, legal guardian or person with medical power of attorney may exercise these rights on the patient's behalf.

Be notified of a breach. We will notify you if a breach compromises the privacy or security of your information.

Changes to this notice

We may change this notice, and the changes will apply to all information we have about you. The revised notice will always be available on this page and at our office, and we will provide a copy on request.
