Referrals
We accept referrals from physicians, psychologists, schools, and families referring their own child. Send the diagnosis you have — confirmed or still being evaluated.
What kind of copy do you need?
Your reference number
Write the number down. It is printed at the bottom of both pages. The form has downloaded to your device — fill both pages, save it, and return it below.
No reference number is issued and nothing is recorded on our side until the completed form reaches us.
How do I fill out the PDF?
- Download or open it.
- Type into the fields — they work in Chrome, Edge, Firefox, Safari, Preview and Adobe Acrobat Reader.
- Save the file. In Chrome that is the download icon inside the PDF viewer, not the browser's print button.
- Return it by upload, email or fax, below.
How will you return the completed form?
Send a form you already have
Upload what you have — an AIMH referral form, a physician order or prescription, a diagnostic evaluation, a school referral, prior reports. You do not need to fill anything else in first. We only need enough to reach you and route the child.

Attach the documents yourself before sending. No web page can attach a file to your email for you — this opens the message, you add the files.

Send, in one fax:
- The completed referral form, or the order or prescription you have.
- The diagnostic evaluation, if one exists.
- The insurance card, front and back.
- A cover page with your name, practice, phone and an email we can reply to.
Write the reference number on the cover page if your copy has one. A fax has no receipt on our side, so give us a phone number we can call back.

- AIMH receives the referral and it enters the intake queue.

- Intake reviews what was sent and checks whether anything is missing.

- We contact the family, and the referral source if we need more information.

- Where applicable, insurance and eligibility are reviewed and next steps are explained to the family.
