What to send, and how.

Referrals come back to us incomplete for the same four reasons. Here they are, so yours does not.

Required

Diagnosis
Documented autism diagnosis from a qualified evaluator, with the date. A working impression is not enough for an authorization.
Medical necessity
Your prescription or letter stating ABA is medically necessary. This is the piece only the referring provider can supply.
Insurance
Plan name and member ID. We verify the benefits ourselves — you do not need to.
Your NPI
Validated at submission. A mistyped NPI is the single most common reason a referral stalls.
Contact
A phone and an email where your office can be reached about this patient.

Helpful, not required

A comprehensive diagnostic evaluation if you have one, the IEP if the child is in public school, and any prior ABA records. These shorten the assessment rather than replace it.

How to send it

Three routes, in order of how fast they move: complete and sign the AIMH-FRM-040 online and attach documents in the same step; fax the signed form to (786) 388-0516; or email intake@americaninstituteofmentalhealth.com and we will send you a secure upload link rather than asking you to attach records to ordinary email.

On documents and privacy

Attachments are uploaded in a separate step from the form fields, stored encrypted apart from the record, and given their own unpredictable address — every opening is logged. We do not put patient information in URLs, and we do not discuss a patient's details by phone with anyone who has not been verified.
